Job Type Full-timeDescriptionUnder the supervision of the Access Program Manager, Clinic Coordinator, and the Public Health Clinical Provider, the Medical Assistant & Health Navigator will perform supportive clinical duties for the medical clinic at Access: Drug User Health in Cambridge, MA. This includes preparing for patient visits, conducting vital sign checks, maintaining clinic flow, and providing support to all clients and medical providers. The MA/Navigator will also provide health education, counseling, and supported referrals for people who inject drugs (PWIDs) who are at high-risk of infection or are HIV/HCV/STI positive, while delivering harm reduction services in a drop-in setting. This includes: providing prevention and screening services to clients who use drugs, syringe exchange, HIV/HCV/STI testing, PrEP and HCV referrals and linkage to care, overdose prevention training, emergency response and overdose response, conflict de-escalation, motivational interviewing, crisis counseling, data collection and individual and group-level interventions aimed to reduce infectious disease transmission and increase access to care and treatment. The MA/Health Navigator will seek to engage and empower clients with a focus on their strengths, dignity, and lived wisdom.Medical Assistant Responsibilities (60%):Provide direct client care, using clinical and office procedures as appropriate for a medical assistant, in an outpatient public health clinic under the direct supervision of the Medical DirectorOrganize and prepare patient visit including conducting client registration, and insurance check in their electronic medical record.Conduct vital sign checksAssist with the triage of incoming medical concerns Assist with exam room turnover, check for general cleanliness and restock supplies as necessary.Conducts inventory for medical supplies, restocks, and rotates stock to prevent expired stock/shortagesAssist with collection and transport of lab samples and specimen collection protocols per DPH and Quest.Maintain clinic flow in a drop-in based outpatient clinic setting.Support client tracking and clinical follow up needsConduct DPH testing sessions for HIV, HCV, Syphilis, Chlamydia, and Gonorrhea evaluating risk through the asking of DPH required questions as well as educating patients on specimen collection instructions for swabs and urine samples.Deliver STD/STI results to clients within 2 weeks of testing session.Assist with the collection and transport of lab samplesAssist with client care coordination needs, including the coordination/delivery of specialty medication prescriptions, prior authorizations, arranging rides to other clinics or community services, medication refills, prescription deliveries. Support projects related to medication adherence improvement or other patient care projects as neededAssist in updating daily temperature check logs for medication room and lab fridges and freezer.Complete appropriate and accurate documentation in a timely manner, including electronic data collection and forms as required.Understands and maintains standard precautions in all clinical activities, uses PPE appropriatelyEnsures CLIA waived testing controls are completed monthly or as needed according to policy and procedures. Ensures CLIA waived testing is adequately stocked.Participate in one-on-one supervision with supervisor, monthly clinical supervision, and team meetings.Effectively communicates with patients, coworkers, and the clinical team across programs/departmentsAttend and participate in AAC/Fenway Health trainings.2. Health Navigation responsibilities (40%):Provide accessible and low threshold services to PWID that operate within the harm reduction framework. Assist PWIDs with accessing support services such as primary care, MAT and behavioral health, homeless shelters, housing and legal advocacy. Connect PWIDs to HCV testing and treatment through relationship building, viral hepatitis education, and recruitment and retention activitiesAssist interested PWIDs with supported referrals, and accompany clients to service sites when appropriate, to address the full spectrum of client-driven needsProvide treatment readiness coaching and counseling to clients in order to facilitate treatment initiation and completionAssist PWIDs with adhering to prescribed HCV treatment medications and host adherence support groups and peer support/skill share groupsConduct risk assessments and risk reduction plans. Provide harm reduction counseling on safer injection and vein care strategies, wound care, overdose prevention, sexual health and HIV/HCV/STI transmission to PWIDs across sites.Train PWIDs in overdose prevention and use of Naloxone. Distribute Naloxone and complete associated MDPH data collection and internal data collection. Collect and distribute syringes from PWIDs. This includes safely handling, storing and disposing of syringes, and general biohazard and infection control protocols Administer Naloxone during overdose events or other emergencies. Work with team to organize daily operations and programs in the drop-in center, including opening and closing procedures to help ensure facilities are clean, safe and welcoming; creating and distributing harm reduction kits; and restocking and organizing supplies in the drop-in centerParticipate in restorative justice protocols and support community advisory boardAttend community meetings as necessary3. Participate in Planning and Implementation of New InterventionsParticipate in program planning through interdisciplinary team meetingsCollaborate with clinical and non-clinical providers to provide quality care and seamless continuum of servicesAttend continuing education seminars, staff in-services, and stay abreast of relevant literature to maintain familiarity with trends in prevention services among people who inject drugs. 4. Meet agency participatory expectations:Adhere to all agency and departmental policies and proceduresParticipate in quality assessment and improvement activities as requestedAdhere to the highest principles of client confidentialityAdhere to established safety policies, procedures and precautions; enforce agency's policies and clinical procedures, including safety and crisis procedures. Attend all required meetings, in-services and professional trainingsMaintains professional competence necessary to perform job responsibilities; maintains and provides agency with records of continuing education activitiesPerform related duties and projects as required Requirements BA/BS or two years harm reduction work/lived experience.Willingness to become competency assessed in basic clinic work, ie vitals collectionCurrent CPR CertificationPossesses significant knowledge of health-related social issues faced by injection drug users.Willingness to be trained in phlebotomy Commitment to practical harm reduction strategies, and drug user health advocacy and rightsStrong organizational, interpersonal, written and verbal communication skills.Experience in conflict resolution, crisis de-escalation, and milieu management. Ability to manage clinically and emotionally challenging situations with utmost professionalism, care and discretion Ability to work effectively with diverse groups of individuals and communities at risk for HIV - especially injection drug users, men who have sex with men, transgender people, people of color, those who are foreign-born and may be undocumented, and those who are homeless or unstably housed. Preferred Qualifications: Phlebotomy certification/experience Completion of a Medical Assistant or EMT programProficiency in Spanish language strongly preferred.Previous experience implementing support groups and trainings.Previous experience conducting street level outreach with at-risk populations a plus.
