Schedule: 5 Shifts per Week | 8 Hours per Shift Pay Rate: $58 – $66
per hour Job Summary We are seeking a licensed Physical Therapist
(PT) to provide high-quality outpatient rehabilitation services.
The Physical Therapist will evaluate, plan, and implement treatment
programs for patients with injuries, disabilities, or medical
conditions, in accordance with physician referrals. This role
requires strong clinical judgment, accurate documentation, and
effective collaboration to support optimal patient outcomes. Key
Responsibilities Evaluate patients’ physical conditions, functional
limitations, and rehabilitation needs Develop and implement
individualized physical therapy treatment plans Administer
therapeutic exercises, manual therapy, and other evidence-based
interventions Monitor patient progress and adjust treatment plans
as needed Maintain accurate, timely, and compliant clinical
documentation Collaborate with physicians, nurses, and
interdisciplinary care teams Complete required clinical and
administrative documentation for reimbursement and compliance
Perform additional clinical or record-keeping duties as assigned
Qualifications & Requirements Bachelor’s degree or higher in
Physical Therapy Active New Hampshire Physical Therapist license
(required) BCLS certification (required) Strong communication and
patient care skills Ability to work independently and as part of a
