Contract Shift Type: Sunday-Thursday, 8:00 AM - 4:00 PM (Day shift;
some on-call/hospice coverage required) Hours per Week: 37

people heal. Roles & Responsibilities Complete accurate OASIS
documentation on time, following agency, state, and federal
guidelines Assess patients and caregivers using the nursing process
to create individualized, evidence-based care plans Serve as case
manager for assigned patients and actively manage caseload and
visit timing Implement medical and nursing interventions to ensure
safe, effective patient care Perform and maintain high-tech skills
such as IV use/maintenance and wound VAC management Provide patient
and caregiver education to support informed decisions and safe
transitions Coordinate interdisciplinary care with physicians,
therapists, social workers, and vendors Document thoroughly in
McKesson EHR, including locking down visits, follow-up notes, and
visit completion Complete required agency and regulatory forms
(HHABN, Medicare Notice of Non-Coverage) accurately and timely
Participate in peer review audits and proactively schedule case
conferences to improve care quality Qualifications Active
Registered Nurse (RN) licensure (per state requirements) Experience
in home health nursing, including care planning and case management
Proven experience with OASIS documentation and timely clinical
documentation practices Comfortable with IV therapy, wound VACs,
and complex wound care Ability to provide hospice/on-call coverage
when assigned Experience working with or delegating to LPNs and
collaborating within a multidisciplinary team Familiarity with
infection control standards and patient/caregiver education best
practices Education Associate or Bachelor of Science in Nursing
(ADN/BSN) or equivalent nursing education Active RN license
required Knowledge & Skills Strong nursing assessment and clinical
judgment skills Skilled in care planning, case management, and
clinical documentation Knowledge of OASIS requirements and home
health regulatory expectations Experience using McKesson EHR or
similar documentation systems Competence in IV therapy, wound VAC
management, and complex wound care Clear communication and patient
education abilities Ability to coordinate with interdisciplinary
