Job Description Certification Details - NH License - N95 Fit Test - MMR - Influenza Vaccine - TB Test - Physician Statement Job Details - Seeking an MLT Generalist to mostly work in the Lab Chemistry department. Job Requirements - NH License required. - Primary Source Verification of professional license and/or certification within 30 days of start date and updated upon expiration. - N95 fit test within 365 days to start date and updated annually. - Documentation of two measles vaccines, two mumps vaccines, and one rubella vaccine or two MMR vaccines or proof of immunity via blood test. - Strongly preferred record of 3 Hep B vaccines or proof of immunity or declination. - Influenza vaccine required during flu season Sept 15th - April 30th, no exemptions. - Documentation of TB skin test screening within 3 months of start date and updated if >6 month break or negative QuantiFERON TB Test or T-spot within 3 months of start date and updated if >6 month break. - Physician statement signed by MD/DO/NP/PA within 365 days of start date and updated if >6 month break. - MGB Criminal Background Check within 45 days prior to start date and updated if >6 month break. - MGB Resume including past 7 years of work experience with city & state. - 2 recent references, minimum 1 Supervisor or Above, in skillset. - MGB Skills Checklist required for submission. - MGB Confirmation of Assignment/POI Form required for all initial assignments and extensions. - MGB Education Verification must verify highest level education achieved. - MGB SSN Trace listing addresses/alias’ where individual lived, worked and/or went to school for past 7 years. - MGB National Sex Offender Check ran within 30 days of start and updated annually. - MGB Exclusion Checks ran within 30 days of start and annually. - MGB New Hampshire State Checks ran within 30 days prior to start date and updated if >30 day break. Schedule Information - 12 week assignment, 5x8 shifts, 40 hours/week. - Evening/Night hours. Manager will discuss shift times during the interview. Unit Specific Information - Mostly work in the Lab Chemistry department. Additional Details - Must live greater than 50 miles from Dover, NH.
