About the RoleMass General Brigham Home Care is seeking a skilled Physical Therapist to provide high-quality care to patients in their place of residence. As a Physical Therapist on our team, you will work collaboratively with patients, families, and healthcare professionals to develop and implement personalized plans of care that promote optimal independence and quality of life.Key Responsibilities

• Conduct thorough assessments of patients' physical impairments, functional limitations, and disabilities to identify areas for intervention.
• Develop and implement evidence-based treatment plans that address patients' unique needs and goals.
• Collaborate with patients, families, and healthcare professionals to coordinate care and ensure seamless transitions between settings.
• Provide education and training to patients and caregivers on physical therapy techniques, safety precautions, and disease management.
• Document patient progress, outcomes, and any changes to treatment plans in a timely and accurate manner.Requirements and Qualifications
  • One year of experience in acute care, rehabilitation, or home care as a Physical Therapist.
  • Registration with the Board of Registration in Massachusetts.
  • Strong customer service skills and ability to work with diverse populations.
  • Excellent verbal, written, and electronic communication skills.
  • Computer proficiency and ability to learn new software.
