Registered Nurse - Pulmonary
Reports to the physician Director of the Multi-disciplinary Lung Nodule and Lung Cancer Screening Program. The nurse navigator serves as a consistent care coordinator to assess the physical, psychological, and social needs of the patient, and to work with the broad health care team to ensure those needs are met. The nurse navigator interfaces with the patient in conjunction with primary care physicians, as well as possible referrals to specialists in radiology, radiation oncology, pulmonology, oncology, and thoracic surgery. Coordinates multi-disciplinary team meetings. Lung nodule management and patient follow-up and assistance through the continuum of care will be an integral part of the position as well to include utilization of nodule management software. The Nurse Navigator may assist with the development, coordination, implementation and evaluation of community outreach events. Works independently to review and assess data related to community health initiatives. May participate in the research and design of community health materials and programs. Independently coordinates and manages specific community programs as assigned. Assists with coordination, implementation and evaluation of community surveys, focus groups and quality improvement projects.
